Critical Incident Support Request

The Mayo Clinic EAP counseling team can provide organizational unit level support for situations that impact a group or team such as an employee death, difficult patient death, or an unfavorable outcome on the floor or unit. You can submit a request by filling out the Critical Incident Support Request form.
